Pursuant to art. 13 para. 1 and 2 of Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 (GDPR / RODO), I have been informed that:

1. The Controller of my personal data is the Buddhist Association Bencien Karma Kamtsang based in Grabnik, Grabnik 4, 96-313 Jaktorów, entered into the register of churches and other religious associations under the number 85, NIP: 1181294140, REGON: 010767828, tel: 663 136 204, e-mail: bencien@gmail.com.
2. Personal data is processed by the Controller in order for the Controller to take action at the request of the author of a message, i.e. contact the author of the message by the Controller in connection with the e-mail having been sent by the author of the message (GDPR, Article 6, paragraph 1, letter b).
3. Personal data may be made available to processors acting on behalf of the Controller, such as providers of IT services.
4. Personal data may also be made available to independent entities, such as the Controller’s co-workers; the Controller’s employees process personal data only for the purpose referred to in point 2).
5. Personal data, processed for the purpose of taking steps by the Controller on my request, shall be stored by the Controller for the period of five years following their collection or until the time when the Controller shall decide about their erasure.
6. I have the following rights:
   1. the right of access to the personal data provided by me and the right to receive a copy,
   2. the right to rectify personal data,
   3. the right to delete personal data,
   4. the right to request a limitation to the processing of personal data,
   5. the right for moving personal data,
   6. the right to object to the processing of personal data provided by me, if they are processed for the purpose of direct marketing,
   7. the right to object to the processing of my personal data for reasons relating to my particular situation, including when the data is processed for a purpose other than direct marketing,
   8. the right to lodge a complaint with the supervisory authority.
7. If the data is processed on the basis of consent, I have the right to withdraw consent to the processing of personal data at any time; the withdrawal of consent shall not affect the lawfulness of processing based on consent before its withdrawal.
8. In order to exercise any of the rights referred to above, I may direct a request in writing to the Controller, to the address: Buddhist Centre Benchen Karma Kamtsang, Grabnik 4, 96-313 Jaktorów, or via email to: bencien@gmail.com.
9. The provision of personal data is voluntary, but in the case the data is not provided, it will be not possible to contact the Controller.
